This convenient step-by-step Action Plan will help you get the most out of your health plan. It outlines key services to schedule, tips to access care quickly, how to get extra support and discounts you may be missing.
USFHP is rated 5 out of 5 for Rating of primary care doctor*. Are you satisfied with your care? Remember: You can switch your Primary Care Manager (PCM) at any time! You can choose a new PCM via HealthLINK, or call Member Services at 800-808-7347.
Did you know? You have top facilities near you! Access premier care, including Johns Hopkins providers and facilities in Maryland and at Sentara, Inova and other leading medical centers in Virginia.
Start by scheduling your preventive care appointments — covered every year.
- Annual Well-Visit — Your yearly Health Promotion and Disease Prevention exams are a chance for your primary care doctor to get to know you, schedule preventive health screenings you may be due to have and create a care plan for the year.
- Annual Eye Exam — Take advantage of your annual no-cost routine eye exam from a SuperiorVision provider, which includes refractions and written lens prescriptions. You also get discounts for certain non-covered services such as lenses, frames and contacts. Schedule 12 months after your last visit.
Find an eye care facility near you - Dental Cleaning — You get two no-cost dental cleanings per year! In addition, you get discounts on dental services at network dentists in most USFHP service areas. Get the details here. Schedule your appointment every 6 months.

After you see your primary care doctor, follow the care plan they set for you.
- Referrals — If you need to see a specialist, you’ll need a referral from your primary care doctor. Make sure the specialist is in-network so you only have a copay for covered services. ADFM copay is $0. USFHP is also rated 5 out of 5 for Rating of specialists*.

Transfer Your Prescriptions
- Walgreens is the Johns Hopkins USFHP contracted pharmacy. If you are prescribed a medication, you’ll need to fill it at Walgreens or through mail order.
Find a Walgreens pharmacy - Be sure to transfer your prescriptions once your coverage becomes effective on Jan. 1.

When you need instant care anywhere for minor but urgent health concerns, use telehealth to have a convenient video chat with a health professional using your phone or computer — no appointment needed! Use anytime during weekends or when your doctor’s office is closed.
24/7 Nurse Chat Line — Have a health concern but you can’t reach your doctor’s office? Call our Nurse Chat Line at no additional cost. A registered nurse is on call 24/7 to answer your questions about an illness, injury or symptom. Call 844-344-4218 to speak with a nurse now.
Our mental health care team can assist you with care coordination for struggles with depression, anxiety, substance use challenges or other mental health conditions. We offer compassionate guidance and useful resources to support your mental well-being journey.
UpLift — Taking the first step to better mental health is not always easy. UpLift is a virtual mental health network that can help make that first step easier. Search for care tailored to your needs and schedule timely appointments — often as soon as one day and no later than two weeks.
