Care Management

As you work toward better health, our care managers are here for you. They help you reach and maintain your best health, and manage your conditions, by offering you individualized support, education, and guidance. They learn about your needs, work with your primary care manager and everyone else on your care team, find resources in the community that can help you, and much more.

Health Education

There is always something new to learn about staying healthy. Johns Hopkins USFHP offers a wide variety of interactive health education programs and workshops to teach you how to take better care of yourself. Topics include managing diabetes and other specific conditions, stress relief, newborn care, weight management, blood pressure, and much more. Best of all, these programs offer a supportive group environment where members can encourage each other and learn from one another’s experiences.

Transitional Care

Coming home from a hospital stay can be both wonderful and a stressful transition. We do everything we can to make it easier for you to adjust. We help you manage medications, teach you about new diagnoses, connect you to community resources, and more.

Nurse Chat

Have a question about your health? Have a health concern but don’t know whether it’s an emergency? Call our Nurse Chat line or use our chat website to speak to a registered nurse about your symptoms, find out where you should go for care, and more. Our nurses are available by phone or online chat 24 hours a day, 7 days a week.

Pregnancy Support

At Johns Hopkins USFHP, we know how life-changing and stressful becoming a mom can be. With our program, we make sure you have the education and tools you need to protect your own and your baby’s health. We teach you what to expect at each stage of pregnancy and offer more in-depth support if needed.

Behavioral Health

Unsure where to start, or need to talk to someone about your emotional well-being? Call your care team. We can help you:

  • Learn more about your mental well-being
  • Help you connect to a specialist
  • Coordinate your care between your providers
  • Offer resources to help you understand and manage your medications

Personal Health Assessment

How can Johns Hopkins USFHP support you?

Could you use a care manager can help you better manage a condition? Are there preventive services that you need?

We encourage you to fill out your Health Risk Assessment (HRA). This questionnaire gives a profile of your overall health and well-being and tells us how we can best serve you.

When you fill out your HRA, it will also help you:

  • Identify existing behaviors that are considered good health practices.
  • Identify protective and health risk factors.
  • Get a comprehensive picture of your health and lifestyle based on current guidelines for optimal health and disease prevention.
  • Make healthy lifestyle decisions.

To complete your HRA, enter your Johns Hopkins USFHP member ID number and the password “wellness2022”.
